ProDispatch Required Paperwork
1. A Completed Service Agreement.
2. A Signed Limited Power of Attorney form. This form is limited to securing freight for you unless
otherwise specified.
3. Completed Company/Carrier Profile. (Your Dispatcher can help with this form)
5. A completed W9 form.
6. References. (can be any company you have worked with. also we do not call references)
7. A copy of your Insurance Certificate. We require the industry standards of $1 Million in Auto Liability
and $100,000.00 in Cargo as a minimum. Please have your insurance agent list ProDispatch;
(Subsidiary of Longhorn Enterprises, llc) as Certificate Holder. (mailing address below if necessary).

2. A completed W9 Form. A W-9 is available for download if you do not have one..

4. A copy of your Insurance Certificate. We require $1 Million in Liability and $100,000.00 in Cargo as a minimum. Please have your insurance agent list ProDispatch (Subsidiary of Longhorn Enterprises, LLC) as Certificate Holder. Mailing address for certificate:
